The SF-86 posted here has extensive error-checking coding included to enable forms to be completed correctly the first time. See the HELP box on the first page of the form for more information. It is highly recommended that you also download the Instructions.

You should sign and date the form ONLY on direction of your DIRAUX. However, as extremely detailed information is required, becoming familiar with the form and its requirements before actually completing it is advised.

Members using the free Reader program cannot, as with any of our forms, save a completed form. You may consider asking a fellow member who has the full Acrobat program to allow you to complete it on their computer. Another alternative is to complete the form at a CG unit, as all CG units have licenses for the full program. You would then be able to copy the completed form to a floppy diskette. These forms will NOT work with Acrobat or Reader versions less than version 5.

Note: In order to use the linking feature built in to the SF-86, you must locate both the Instructions for Completion of SF-86 and the SF-86 in the same folder on your computer.
