This page will be maintained to keep our EFORMS users informed on any changes made to the forms posted on this site. We will list each form by ANSC form number (or "Approved Form Number"), title, and revisions to form files, as they are posted.
This listing will include all form changes, including base form (blank) changes and form field changes. If the form change obsoletes a previous form date, which can occur only when the actual form date is changed, a notice will be posted both here, and on the National Website's "What's New" page. If the revision does not affect the form date, or if the previous form may still be used, the announcement will be included here, but not necessarily on the "What's New" page.
NOTE: To see the form's "Revision Number", which refers to the Acrobat PDF file revision, open the form using Reader. Press Ctrl-D to display the document information, and view the Title: box for the Rev-xx entry. The last modification date of any form is listed as the Modified: date.
Update Acrobat Reader to Use Newer Forms

Acrobat Reader 5 and Acrobat Approval seem to have compatibility issues with revised forms. Members are advised to go to the Adobe website and download a later version of reader - a free download.
NEW 7029 POSTED ON FORMS PAGE

A new 7029H-Jan 2010, has been posted on the forms page. The old 7029G will NOT be able to be entered into AUXDATA after Sunday, 3 January 2010. The new form will be necessary. The eForm version will be posted as soon as possible.

REVISED FORMS

The Member Activity Log, ANSC-7029 (02-05) and the Mission Activity Report, ANSC-7030 (04-09) have been posted without instructions. These forms are in addition to the current ANSC-7029 and ANSC-7030 and are intended for electronic filing using email.
NOTE: (The following feature is not available with Adobe Reader. This is an Adobe limitation not an Auxiliary one.)
These forms can be saved and reused. This will save you from having to reenter data (i.e.: Member ID, Name, Division, Flotilla, etc.) that does not change over time. Simply open a saved form (i.e: my documents/7030-mission1.pdf), make your edits, save with a new name (i.e: my documents/7030-mission2.pdf), then submit the new form via email.
